Winners must agree to sign the queen’s contract, parent’s contract, notarized health/liability form.  Winners/attendants & a parent must

attend the photos & meeting at  7 PM on June 24th.

 

 

 

 

Print and mail completed application with a copy (not original) of last report card (birth certificate for 5 year olds not in school) and $10 registration fee.  $10 entries must be received by mail no later than June 16, 2008.  Not responsible for lost or late mail.  Any mailed applications received after June 16th will be returned to sender.  Applications submitted at the June 16th meeting require an extra $10 on site fee for a total of $20.  Make money order payable to London Strawberry Festival Queens’ Program.  NO CASH or CHECKS.

All applications must be received by June 16th.  Allow at least a week for mailing time.
