Winners must agree to sign the queen’s contract, parent’s contract, notarized health/liability form. Winners & a parent must

attend the photos at 6:30 & 7 PM meeting on June 26^th.

Print and submit completed application with agreement to queen’s contract & parent’s contract, copy of last report card and registration fee. Little Miss applicants who have not yet started school must submit a copy of their birth/hospital certificate. $10 Early Bird entries must be received by mail no later than June 18, 2006. Mailed applications received after June 18^th will be returned to sender. Applications submitted at the June 18^th meeting require an extra $10 on site fee for a total of $20. Make check or money payable to London Strawberry Festival Queens’ Program. NO CASH
